Study note

Delivery decisions in imminent eclampsia

THE CONTEXT
The constant threat of eclampsia makes maternal interest central to decisions about delivery.

When to consider delivery

•
Delivery should be seriously considered when pregnancy is beyond 37 completed weeks.
•
If the condition fails to improve within a reasonable period, stated as 6–8 hours, delivery should be seriously considered irrespective of gestational age.

Method of delivery and preparation

•
Delivery may be achieved by low rupture of the membranes aided by oxytocin infusion or by cesarean section, depending on disease severity and the state of the cervix.
•
Prostaglandin E2 gel may help cervical ripening.
•
Corticosteroid is given when pregnancy is less than 34 weeks.
