Study note

Device therapy and transcatheter repair for mitral regurgitation

THE CONTEXT
Management options include cardiac resynchronisation in eligible heart failure patients and percutaneous repair to improve leaflet coaptation.

Ventricular function and resynchronisation

•
When ventricular dilatation causes mitral regurgitation, valve repair or replacement may worsen ventricular function because the ventricle can no longer empty into the low-pressure left atrium.
•
In eligible heart failure patients, cardiac resynchronisation therapy pacemakers can improve left ventricular function and dilatation and reduce functional mitral regurgitation.

Transcatheter edge to edge repair

•
Percutaneous transcatheter edge to edge repair (TEER)* can improve leaflet coaptation, especially in patients unsuitable for cardiac surgery.
