Surgery
The thyroid gland
Diffuse hyperplastic goitre and its colloid stage
THE CONTEXT
Diffuse hyperplasia represents the early stages of simple goitre and can change with thyroid-stimulating hormone stimulation.
Presentation and course
•
Diffuse hyperplastic goitre appears in childhood in endemic areas and usually at puberty in sporadic cases, when metabolic demands are high.
•
The goitre is soft and diffuse and may enlarge enough to
cause discomfort.
•
It may regress when thyroid-stimulating hormone (
TSH
) stimulation ceases, but can recur during later stress, such as pregnancy.
Colloid goitre
•
Colloid goitre
is a late stage of diffuse hyperplasia, occurring after TSH stimulation has fallen.
•
Many follicles become inactive and filled with colloid.
