Study note

Insufficient breast milk: causes and remedies

THE CONTEXT
The passage links feeding practices and maternal problems with measures to support milk supply.

Establishing and maintaining feeding

•
For delayed initiation, start breastfeeding immediately after vaginal delivery and within 30 minutes after cesarean section, with early skin-to-skin contact.
•
Avoid supplementary feeding and encourage frequent night feeding when night feeds are infrequent.
•
To maintain optimum lactation, express milk from both breasts 6 to 8 times in 24 hours.

Breast and nipple problems

•
For cracked or sore nipples, correct positioning and attachment, apply hindmilk, and provide oral analgesics.
•
For breast engorgement, correct positioning and attachment and use hot fomentation, massage, frequent breastfeeding or milk expression, and oral analgesics.

Maternal support

•
Address maternal stress with counseling.
•
Provide specific treatment and pain alleviation for maternal illness or painful conditions.
•
When sleep is insufficient, ensure proper sleep and rest and keep the neonate and mother together.
