Study note

Long-term health outcomes after severe pre-eclampsia

THE CONTEXT
Severe pre-eclampsia is associated with risks in subsequent pregnancies and later cardiovascular, renal, and metabolic health.

Subsequent pregnancy outcomes

•
Future pregnancies carry increased risks of pre-eclampsia, placental abruption, fetal growth restriction, and spontaneous or indicated preterm birth.
•
Women with HELLP syndrome have a higher risk of recurrence.
•
Recurrent pre-eclampsia is listed at 25%.

Later health outcomes

•
Cardiovascular outcomes include chronic hypertension, cardiomyopathy, ischemic heart disease, and coronary artery disease. Cardiovascular disease risk is increased later in life.
•
Renal outcomes include glomerular dysfunction and chronic renal disease.
•
Retinal detachment, type 2 diabetes, and obesity are also listed among remote health outcomes.
•
Metabolic syndrome is included; the stated common underlying pathologies are thrombophilias, insulin resistance, or preexistent renal disease.

Risk-reducing interventions

•
The passage lists a healthy diet, avoidance of smoking, weight loss, and aspirin therapy as interventions that improve outcomes.
•
Management of hypertension, type 2 diabetes, thrombophilias, and dyslipidemia is also included.
