Study note

Management of myocarditis

THE CONTEXT
Treatment is primarily supportive, with management of cardiac complications and escalation of circulatory support when medical treatment is inadequate.

Supportive treatment and activity

FeatureDetails
Cardiac complications
Treatment should be given for cardiac failure or arrhythmias.
Physical exertion
Patients should avoid intense physical exertion because some evidence suggests it can induce potentially fatal ventricular arrhythmias.
Immunosuppressive treatment
There is no evidence of benefit from glucocorticoids and immunosuppressive agents.

Specific treatment and circulatory support

FeatureDetails
Antimicrobial therapy
Specific treatment may be used if a causative organism has been identified, but this is rare.
Mechanical support
Patients responding inadequately to medical treatment may temporarily require a mechanical ventricular assist device for circulatory support.
Transplantation
Cardiac transplantation may rarely be required.
