Study note

Management of pre-eclampsia with severe features

THE CONTEXT
The interpretable branches of the management scheme distinguish stable pregnancies before 34 weeks from cases requiring immediate delivery.

Stable pregnancy before 34 weeks

•
Pre-eclampsia with severe features before 34 weeks requires admission and monitoring.
•
When maternal and fetal conditions are stable and persistent symptoms are absent, the scheme advises trying to continue pregnancy to at least 34 weeks, if not 37 weeks.

Indications for immediate delivery

•
Pre-eclampsia with severe features and indications for immediate delivery requires delivery irrespective of gestational duration.

Supportive measures and monitoring

•
The scheme lists counseling of the couple and transfer to a tertiary care facility with a neonatal care unit.
•
Prophylactic magnesium sulfate therapy, antihypertensive therapy, and steroid therapy when pregnancy is less than 34 weeks are listed.
•
Postpartum monitoring is included in the management scheme.
