Management varies with the cause of regurgitation, pulmonary artery pressure, valve damage and whether mitral valve surgery is planned.
Response to treatment and tolerance
•
Tricuspid regurgitation caused by right ventricular dilatation often improves when cardiac failure is treated.
•
Patients with normal pulmonary artery pressure tolerate isolated tricuspid regurgitation well.
•
Tricuspid valves damaged by endocarditis do not usually require replacement.
Surgical repair and replacement
•
Patients undergoing mitral valve replacement who have tricuspid regurgitation from marked tricuspid annular dilatation benefit from repair with an annuloplasty ring.
•
The annuloplasty ring brings the valve leaflets closer together.
•
Rheumatic valve damage may require tricuspid valve replacement.
Transcatheter repair
•
Percutaneous transcatheter edge to edge repair (TEER) improves leaflet coaptation, reduces tricuspid regurgitation and improves quality of life.
•
Similar TEER procedures are available for mitral regurgitation.
