Neonatal HIE: evolving injury, staging, and prognosis
THE CONTEXT
The passage describes the continuing course of hypoxic injury and the value of HIE staging for anticipating disease evolution and long-term outcomes.
Evolution and manifestations
•
Hypoxic injury begins at the onset of the insult, continues after resuscitation, and can manifest as sequelae in childhood.
•
Clinical manifestations vary with injury severity.
•
HIE in term neonates
is described using staging systems.
•
Preterm HIE manifests with abnormalities of consciousness, tone, and seizures, but has no distinct classification system.
Stage-related prognosis
•
HIE staging helps predict disease evolution and long-term outcomes.
•
Stage 1 HIE
has a uniformly good prognosis.
•
Adverse neurological outcomes
occur in 20% of neonates with stage 2 HIE.
•
In stage 3 HIE, half of neonates die, and the surviving half have poor neurodevelopmental outcomes.
