Study note

Prognostic factors in coronary artery disease

THE CONTEXT
The burden of coronary artery disease (CAD) and the degree of left ventricular dysfunction determine prognosis.

Disease extent and ventricular function

•
Single-vessel disease with good left ventricular function is associated with five-year survival above 90%.
•
Severe left ventricular dysfunction with extensive three-vessel disease is associated with five-year survival below 30% unless revascularisation is performed.

Symptoms and mortality

•
Symptoms are a poor guide to prognosis overall.
•
Nevertheless, patients with severe angina, classified as CCS angina scale III or IV, have nearly twice the five-year mortality of patients with mild symptoms.
