Study note

Status eclampticus: treatment escalation and intubation

THE CONTEXT
The passage describes treatment for status eclampticus, escalation when treatment fails, and indications for intubation.

Thiopentone administration

•
Thiopentone sodium 0.5 g is dissolved in 20 mL of 5% dextrose and administered intravenously very slowly.
•
The procedure should be supervised by an expert anesthetist.

Escalation in unresponsive cases

•
If the procedure fails, complete anesthesia, a muscle relaxant and assisted ventilation may be employed.
•
In unresponsive cases, cesarean section in ideal surroundings may be a lifesaving attempt.

Indications for intubation

•
Indications include persistent unconsciousness in the post-seizure period, signs of aspiration, persistent hypoxia and uncontrolled seizures.
