Thyroid function testing in discrete thyroid swellings

THE CONTEXT

Biochemical assessment identifies whether a discrete thyroid swelling is associated with hyperthyroidism and guides functional imaging.

•

Measure serum TSH and thyroid hormone levels.

•

Biochemically confirmed hyperthyroidism with a discrete swelling indicates either a toxic adenoma or a manifestation of toxic multinodular goitre.

•

The combination of toxicity and nodularity is an indication for isotope scanning to localize the areas of hyperfunction.
